Documentation Standards
How your sources should be documented on your submissions.
Name Submission Form Checklist (pdf)
Follow this list to make sure your name submission won't suffer the dreaded Administrative Return.
Name Books That Do Not Require Photocopies Being Appendix H of the Administrative Handbook.
You do not need to send photocopies of your name sources if they are included on this list. However, you still need to include the header name and page number and edition of the book in which the reference name is found.
